Licensed Nursing

At home assurance of highly qualified care

Your doctor may prescribe follow-up care after an illness or injury. Our registered nurses or therapists can work with your doctor to develop a tailored health care plan that addresses your medical needs while maximizing your personal comfort and independence. We pride ourselves on our ability to be responsive, providing services within 24 hours of a referral.

Medicare has rated Porter Hills Home Care’s Home Health Services five (5) out of five (5) stars*! A 5 star rating places an agency within the top 3.6% of agencies nationwide.

A Registered Nurse or Therapist will conduct a preliminary assessment of your healthcare needs. In coordination with your personal physician and family, we can help you with a range of services at home such as:

  • Treating an illness, such as pneumonia or an infection
  • Managing a disease, such as diabetes
  • Providing specialized treatments and procedures, e.g., for Parkinson’s disease or respiratory issues
  • Treating a wound
  • Offering health and nutritional education
  • Therapy following illness, surgery or injury

Licensed Medical Social Workers (LMSW) advise you on ways to stay in your home and maintain your independence as you age and if your health changes. These professionals teach coping skills and stress management techniques, and find resources to meet your needs.

  • Community Resources
  • Long-Term Care planning
  • Short-Term Therapy (depression, anxiety and grief and adjustment)

Psychiatric Nursing (PSN) are experts in crisis intervention, mental health, and medication management. They work closely with you so that you can live your best life.

Home health aides make it possible for you to continue receiving in-home health care by helping with such activities as bathing, shampooing, grooming, exercises, ambulation and meal preparation, on a short-term basis, all under the supervision of a registered nurse or therapist.
