A Conference on Collaboration: Care is Better Together!
1000 East Beltline Ave NE
Grand Rapids Charter Township, MI 49525
Long term community support and services for elders. Challenges and opportunities for the frail in West Michigan. Community discussion facilitated by Altaram. New objectives and collaborations based on prioritized goals.
Without significant structural changes in service delivery, an aging nation faces a future of substantial costs and needless pain and distress among the elderly.
Join us in exploring how our community can safely, securely and affordably meet the needs of West Michigan elders living with challenges to their health and well-being through the collaborative efforts of health systems, expert providers and trusted ambassadors for the betterment of the community.
Together we can meet the healthcare needs of elders in West Michigan by a community of providers, in the community, for the community.
Presented by Tandem365, a partnership of Clark, Holland Home, Life EMS, Porter Hills and Sunset.
