Skilled Nursing and Rehabilitation
As needs change, Porter Hills provides the highest level of care available to seniors outside of the hospital. For individuals needing longer stays, we create a warm and positive environment in which our patients thrive.
As a faith-based organization, our goal is to help those we serve maintain their quality of life in a warm, positive and caring environment. Full time, long-term skilled nursing is provided to our residents at the Health & Rehabilitation Center at Porter Hills Village. The staff includes registered nurses, physical, occupational and speech therapists as well as activity directors and social workers. Rooms are private or semi-private.
- Round-the-clock nursing care
- Significant assistance with the activities of daily life
- Nursing staff on duty 24 hours a day to help individuals meet their daily physical, social and psychological needs
- A licensed physician supervises each resident’s care and therapy including speech, occupational and physical therapy
- On staff licensed social workers consult regularly with resident and families to develop a Care Plan
Pharmacy Service is provided by a contracted pharmacy and delivered to Porter Hills. Medication monitoring and distribution is provided by a trained staff member and customized to residents’ individual needs. Residents benefit from having prescriptions and over-the-counter medications supplied for them in the convenience of their apartment.
The Porter Hills Chaplain links residents with local churches if desired, provides worship opportunities for those residing in the health care settings and is available to meet with residents at their request for spiritual counsel.
The Medical Director communicates directly with our nurses and staff to provide residents with excellent medical care. These services are billed to Medicare and the resident’s personal insurance company.
The team provides each resident with personal care, supervision, and direction. Each member is trained to utilize effective communication skills that benefit those living in our community. Staff members meaningfully engage residents and are involved in developing and promoting positive interactions, and group and individual activities/projects that encourage resident decision-making.
