Care Coordination

We know that making arrangements for post-hospital care is sometimes difficult.

At Porter Hills, our residents receive personal assistance in coordinating post-hospital-stay services such as home health care, rehabilitation, or skilled nursing.  A community liaison visits each resident who is hospitalized, and works with his/her family to help ease the transition from hospital to home.
