Health Monitoring
Porter Hills in-home health monitoring assists you and your doctor with the management of your illness. A monitoring device allows you to check your vital signs and symptoms in the comfort of your own home, and then use your home telephone line to send that data to our specially trained Registered Nurse for evaluation.
Health monitoring is intended to:
- Avoid emergency room visits and decrease hospital re-admission
- Enhance home care service
- Encourage client participation in their personal health maintenance
A physician’s order is required to implement an in-home health monitoring plan of care. Please contact your physician to determine if health monitoring is appropriate for you and if you are eligible under Medicare. Health monitoring is also available as a private pay service
