Assisted Living services are offered in a distinct area of Meadowlark that is licensed Adult Foster Care. Staff interact with residents throughout the day to ensure appropriate support services are provided.
In addition to quality living environments, the following services are included in the Assisted Living monthly rate:
- Breakfast, lunch and dinner daily
- Staff assistance and observation with medications, with Licensed Nurse oversight
- Weekly housekeeping services
- Assistance with personal hygiene, grooming, dressing and bathing
- Reminders and cueing for memory loss and confusion
- Laundering clothing and linens (weekly)
- Stand by assistance with mobility or wheelchair transportation to activities and meals
- Room furnishings (if necessary)
Monthly fee for both Residential and Assisted Living include:
- Daily staff interaction
- Nurse support in determining needs, setting health-related appointments, answering health questions and scheduling of personal and supportive services
- Staffed 24 hours per day
- Emergency staff response
- Social, recreational and spiritual programs
- Evaluation of vital signs once a month (optional)
- Heat, electric and water/sewer
- Air-conditioned halls and community rooms
- Cable television
Amenities
Lab Service provides residents the ability to request on-site lab work. The contracted lab provides services at Meadowlark and returns the results to the resident's ordering physician.
Pharmacy Service is provided by a contracted pharmacy and is delivered to Meadowlark for the residents. Medication assistance is provided by trained staff and overseen by our licensed nurse. The residents benefit from the convenience of having prescriptions supplied for them without requiring transportation.
Transportation Services are available through Sparta Senior Neighbors and North Kent Transit to Grand Rapids and the Northern Kent County area. Appointments are required and fees may vary. Also, Meadowlark staff provides grocery service to residents and the cost of groceries is included on the resident's monthly statement (a minimal fee for this service may apply).
Spiritual Services are provided on a weekly basis. Also the Porter Hills Chaplain's services are available upon request. The Chaplain makes regular visits to Meadowlark and visits residents who are hospitalized. Spiritual counsel is also available.
Porter Hills Medical Director is available to become a resident's personal physician and visit residents at Meadowlark. These services are billed to Medicare and the resident's personal insurance company. The Medical Director has a unique ability to communicate directly with our nurse and staff to provide residents with excellent medical care.
