Porter Hills Village

1961 Planning committee formed
1968 Ground breaking ceremony
1970 Porter Hills Village opens in Grand Rapids Township offering residential living and skilled nursing
1972 Town home residential living added
1974 Assisted living services developed
1982 Garden Apartments West residential living added to community
1986 Harmony Hall opens for Alzheimer's care
1988 Garden Apartments East residential living added to community
1992 New Health Center wing completed, Generations Child Development Center opened and chapel added to community
1993 Received national accreditation from the Continuing Care Accreditation Commission (CCAC), the first in West Michigan
1996 Assisted living services expanded
1998 CCAC re-accreditation achieved
2003 Campus renovation begins
2004 CCAC re-accreditation achieved
2004 Additional town homes planned
2005 35th Anniversary
2005 Construction begins on new Town Homes
2005 Agreement signed with Hospice of Michigan
2006 Blessing of residential hospice in the Health Center in collaboration with Hospice of Michigan
2007 Designated rehabilitation area established in the Health and Rehabilitation Center
2007 Sculpture dedication for "A Life that Shines" in honor of

2008 Blessing of the ground for two 10-room GREEN HOUSE homes in front of the Cook Valley Estates campus
2009 First residents move into The Green House homes from the Health Center at Porter Hills Village
2009 Award of Excellence by ABC (Associated Builders and Contractors, Inc.) in the under $5 million category for Outstanding Workmanship and Overall Quality of Construction in Green Building

2000 Opens in Grand Rapids Township offering estate homes and residential living apartments
2002 Additional estate homes added to community
2004 Residential living town homes planned
2006 Meijer-Hadley Wellness Center opens
2009 MMPC opens physician clinic on campus

Meadowlark Retirement Village

1997 Purchased Meadowlark Retirement Village in Sparta offering residential living plus services and assisted living
2004 Additional assisted living added to community

Affordable Housing Communities

1997 Walker Meadow opens in Walker
1998 Harvest Way opens in Sparta
1999 Oak Ridge opens in Grand Rapids
2000 River Grove opens in Belmont
2002 Bailey's Grove opens in Kentwood

1997 Home Health Care and Private Duty agencies established
1999 Martindill Wellness Center opens
2000 Porter Hills Rehabilitation established
2004 CHAP accreditation for Home Health and Private Duty
2005 Sunshine Senior Assistance acquired
2005 Michigan Award of Excellence for Improving Care in the Home Health Setting for Home Health Care
2007 Plus Care program begins offering care to residents of Villa Maria
2008 Sunshine Senior Assistance becomes Sunshine Transportation
2009 Agreement signed with Pilgrim Manor/Pilgrim Care to provide nursing services
